This game is being played with Fate, so this sort of narrative “trick” is supported in a number of ways by the game system itself. In this case, Hollywood could have rolled an attack against Meltdown, and the above is how it played out, or Hollywood could have rolled for an advantage (the hologram trick) before attacking, among other plausible ways. No need for a DnD-esque rule to outline this exact setup.
Fate character creation also involves creating aspects for your character, so more then likely one for Hollywood was something ranging from a generic Someone wants me dead and Carlos and whoever is playing Hollywood (I have unfortunately forgotten) worked out the details with meltdown, to a more specific Meltdown wants me dead for killing her wife, Meltdown was probably introduced or established in one of the character creation stories assuming they used standard character creation rules and didn’t change it alongside the cards very possibly the specific story of Hollywood killing her wife.
I wonder if “they made me do it” refer to killing Meltdown’s wife, which would imply that she may not have deserved it, or if Hollywood is saying that about him just ripping open the wound to twist the knife in order to get Meltdown mad?
If it comes up in game, yes. Could be the result of an interrogation played out as a verbal/social fight. As each side draws cards, portions of the backstory are created based on those draws.
Sounds like Hollywood might have a Meltdown after this mission is over.

A well written villain is one the reader will side with or feel bad for. This is difficult for most writers to pull off. You did it amazingly well.
